How to flag an evidence, identity, source, regulatory or wording issue for correction.
What to report
Outdated regulatory status, broken or incorrect source links, compound-identity errors, formulation/route problems, missing negative findings, or wording that overstates the evidence.
What makes a useful correction
Include the affected page, exact sentence or field, what you believe is wrong, and a primary source or regulator/registry link where possible.
How corrections are treated
Material corrections should be dated and visible through Latest Evidence / Change History rather than silently rewriting the record.
